What is Microtia?
Microtia is a rare birth condition where a baby is born with a small or underdeveloped outer ear. The word itself comes from Latin — “micro” meaning small and “otia” meaning ear. Being a congenital condition, it develops during pregnancy, most often in the first trimester, when the ear structures do not fully form.
The severity of microtia can vary. In mild cases, the ear may be smaller than usual but still recognizable, while in severe cases, parts of the ear may be missing. In very rare situations, the external ear is completely absent, a condition known as anotia. Sometimes, children with microtia are also born without an ear canal, which can lead to hearing difficulties and problems detecting where sounds come from.
Most often, microtia affects only one ear (unilateral), but in about 20% of cases, both ears (bilateral) may be involved. It is also more common on the right side of the head than the left, and studies show it occurs more frequently in boys than in girls. The condition is seen in approximately 1 out of every 6,000–7,000 births and is more prevalent among Hispanic, Asian, and Native American populations.
Children with microtia often experience partial hearing loss due to abnormalities in the outer or middle ear. However, the inner ear, which plays the main role in hearing, usually develops normally. With the help of hearing aids or surgical options, many children can overcome these challenges.
Microtia may also occur as part of other developmental conditions, such as hemifacial microsomia, where multiple facial features do not develop fully. Depending on the type, children may have only an earlobe, a partially formed conchal cavity, or an ear canal that doesn’t function properly.
Can You Still Hear with Microtia?
Yes, many people with microtia can still hear, but the extent depends on how the ear has developed. Since microtia often affects the outer and middle ear, some degree of hearing loss in the affected ear is common. If the ear canal is missing or blocked, sound cannot travel in the usual way. However, the inner ear, which is responsible for processing sound, usually functions normally. This means that hearing is often possible, even if the ear canal is closed.
In some cases, surgery to create or open the ear canal can improve hearing. For children who cannot undergo surgery, modern hearing devices such as bone-conduction hearing aids can help transmit sound effectively.

What to Expect During the Surgery
Microtia surgery is usually performed when a child is around 6–10 years old, once the ear cartilage and facial structures have developed enough. The procedure may vary depending on the surgical technique, but here’s what you can generally expect:
- Anaesthesia – Your child will be given general anaesthesia to ensure they are fully asleep and comfortable during the operation.
- Ear framework creation – Surgeons often use cartilage from the child’s ribs (or sometimes synthetic implants) to build a natural-looking ear structure.
- Shaping the ear – The new framework is carefully sculpted and placed under the skin where the ear should be, giving it a natural shape and contour.
- Multiple stages – Depending on the technique, the surgery may be performed in a single stage or in 2–3 stages spread over several months.

What are the causes of microtia?
The exact cause of microtia is still not fully understood. In most cases, it happens without a clear reason, and nothing a mother did during pregnancy could have prevented it. Microtia develops early in pregnancy, usually in the first trimester, when the outer ear is forming. If something disrupts this process, the ear may not develop normally.
While the majority of cases have no known cause, research has identified a few possible factors that may increase the risk:
- Genetic changes – Sometimes, microtia may occur due to mutations in a single gene or as part of a hereditary condition.
- Maternal health during pregnancy – Infections such as rubella during pregnancy can interfere with ear development.
- Vitamin A imbalance – Abnormally high or low levels of vitamin A during pregnancy may affect normal ear formation.
- Certain medications – Drugs such as isotretinoin (Accutane®), used for severe acne, have been linked to congenital anomalies, including microtia.
- Hereditary conditions – Microtia may also be one feature of syndromes like hemifacial microsomia, where multiple facial structures are underdeveloped.
Despite these associations, in most children with microtia, the cause remains unknown.
