What is Interstitial Lung Disease?

Interstitial Lung Disease (ILD) refers to a group of nearly 100 chronic lung disorders that primarily cause inflammation and progressive scarring (fibrosis) of lung tissue. This scarring thickens and stiffens the lungs, reducing their ability to expand fully and take in oxygen. Over time, this makes breathing increasingly difficult and limits the amount of oxygen reaching the bloodstream.

In most cases, people with ILD experience shortness of breath, especially during physical activity, and may have a persistent dry cough. The condition can be caused by prolonged exposure to harmful substances such as asbestos, silica, or coal dust, or may develop as a complication of certain autoimmune diseases like rheumatoid arthritis. In some cases, however, the exact cause remains unknown.

The inflammation associated with ILD can affect different parts of the lungs:

  • Bronchiolitis – inflammation of the small airways (bronchioles)
  • Alveolitis – inflammation of the air sacs (alveoli) where gas exchange occurs
  • Vasculitis – inflammation of the tiny blood vessels (capillaries) in the lungs

Once fibrosis (scarring) occurs, it cannot be reversed. Treatment focuses on slowing further lung damage, easing symptoms, and improving quality of life. Depending on the type and severity of ILD, medications may help reduce inflammation and slow disease progression, but some people may eventually require a lung transplant.

The course of ILD varies from person to person; it may remain stable for years, progress slowly, or worsen rapidly. Symptoms can range from mild to severe, making regular monitoring and timely medical care essential.

Symptoms of Interstitial Lung Disease

The symptoms of ILD can develop gradually over months or years, or in some cases, progress quickly. They often worsen over time as lung scarring increases.

Common symptoms include:

  • Shortness of breath (dyspnoea) – especially during physical activity and, in advanced stages, even at rest
  • Persistent dry cough – a long-lasting cough without mucus production
  • Fatigue and weakness – due to reduced oxygen supply to the body
  • Chest discomfort or pain – a dull ache or tightness in the chest
  • Unexplained weight loss – often linked to chronic illness and reduced appetite

Other possible symptoms:

  • Clubbing of fingers or toes – rounded, widened fingertips caused by low oxygen levels
  • Bluish tinge to lips or skin (cyanosis) – from decreased oxygen in the blood
  • Swelling in legs or ankles – can occur in advanced cases if the heart is affected

Causes of Interstitial Lung Disease

Interstitial lung disease can result from a variety of factors that damage the lung tissue, leading to inflammation and scarring (fibrosis). In some cases, the cause remains unknown, which is referred to as idiopathic ILD.

  1. Long-term exposure to harmful substances
    • Repeated inhalation of irritants over time can injure lung tissue. Common examples include:
      • Asbestos fibres – often found in old building materials
      • Silica dust – from mining, sandblasting, or construction
      • Coal dust – from coal mining
      • Grain dust, mold, or animal proteins – in agricultural settings
      • Metal dusts – from welding or industrial work
  2. Autoimmune and connective tissue diseases
    • Certain chronic inflammatory conditions can damage lung tissue, such as:
      • Rheumatoid arthritis
      • Systemic lupus erythematosus (lupus)
      • Scleroderma
      • Polymyositis or dermatomyositis
      • Sjögren’s syndrome
  3. Certain medications and treatments
    • Some drugs and therapies can cause lung inflammation or scarring, including:
      • Chemotherapy drugs
      • Heart medications
      • Certain antibiotics
      • Radiation therapy to the chest
  4. Infections

    Some viral, bacterial, or fungal infections can trigger lung inflammation that leads to ILD.

  5. Unknown (Idiopathic) causes

    In many patients, no specific cause can be found. The most common idiopathic form is Idiopathic Pulmonary Fibrosis (IPF), which tends to worsen over time without a clear reason.

What to Expect During ILD Evaluation

  1. Medical history and symptom review

    Your doctor will ask about your breathing problems, cough, and how long you’ve had symptoms. They will inquire about exposure to dust, chemicals, or mold, smoking habits, travel history, family history of lung disease, and any existing autoimmune conditions.

  2. Physical examination

    Using a stethoscope, your doctor will listen for crackling or “Velcro-like” sounds in your lungs. They may also check your oxygen levels using a pulse oximeter.

  3. Imaging tests
    • Chest X-ray – to detect lung abnormalities.
    • High-Resolution CT (HRCT) scan – provides detailed images to spot scarring or inflammation.
  4. Lung function (Pulmonary function) tests

    Measures how well your lungs take in and release air, and how efficiently oxygen moves into your blood. Common tests include spirometry and the diffusing capacity test (DLCO).

  5. Blood tests

    Help rule out infections and detect autoimmune or inflammatory diseases.

  6. Oxygen measurement

    A 6-minute walk test may be performed to see how your lungs respond to physical activity.

  7. Bronchoscopy or lung biopsy (if needed)

    In some cases, a small sample of lung tissue is taken to confirm the diagnosis and identify the type of ILD.
